IDTA Report forms
Following an examinations session, Head Office sends a small stock of replacement report forms with the Awards to the school principal. The correct report forms must be used for the various IDTA examinations, if you do not have the correct report form or you are unsure about which form to use please contact the office to request the appropriate forms, email info@idta.co.uk or phone 01273 685652. Please allow enough time before your examination session for any additional reports you need to be posted to you. When you send in your Examination Session Request Form, please check to see if you need any report forms and include a note with the Request Form asking for a stock of forms to be sent to you.

Please note, all professional report forms are sent direct to the Examiner by Head Office (Associate, Licentiate, Fellowship, Diplomas and Level 4 Diploma in Dance Teaching).
Also, the Ballroom Branch Professional Performance Award reports are sent direct to the Examiner by Head Office. |
